14. Dr. Browne asked the Minister for Social Welfare if he will make SW 54 forms readily available to the public through dispensaries, post-offices and so on.

Mr. Haughey: Supplies of the information leaflet SW 54 which gives [563] details of the Supplementary Welfare Allowances Scheme have been distributed to almost a thousand health centres throughout the country. The leaflet is also available at all information offices of my Department and at the community information centres registered with the National Social Service Council.
